Community Support Program (CSP)

Individual CSP Services are provided in coordination with Adult Mental Health Case Management Services to SPMI adults in need of additional supports to live successfully in the community.

Individual CSP services assist adults with serious and persistent mental illness (SPMI) in the following ways:
  • Providing outreach services, such as home visits and wellness checks
  • Assisting consumers with:
    • Problem-solving and connecting with mental health and other resources
    • Accessing community resources and supports
    • Applying for health insurance and other benefits
    • Developing a healthy lifestyle and independent living skills, such as:
      • Shopping
      • Budgeting
      • Household management
      • Cooking and nutrition
  • Accessing employment resources and supports
  • Taking medications as prescribed
  • Consistently attending appointments
  • Developing interpersonal communication and social skills
  • Educating themselves about mental illness, treatments, and recovery
  • Developing relapse prevention and symptom management skills
