Adult Mental Health Case Management Services assist adults with a serious & persistent mental illness (SPMI) to:
- Identify their mental health and other service needs, develop their personal goals, and access needed services including:
- Mental health
- Medical / physical health
- Financial
- Vocational
- Social
- Housing
- Transportation
- Advocate for their service needs
- Educate themselves on management of their illness
*Adult Mental Health Case Management Services are also referred to as:
- Rule 79 Case Management Services
- Targeted Case Management Services
Intake Services
Adult Mental Health Case Management Intake Services assist individuals requesting Adult Mental Health Case Management services by:
- Working with individuals to arrange a diagnostic assessment to establish their eligibility
- Providing interim case management services to individuals seeking Adult Mental Health Case Management eligibility
- Providing individuals with information on mental health and other community resources
- Assisting individuals with referrals for needed services
