Adult Mental Health Case Management Services
Adult Mental Health Case Management Services assist adults with a serious & persistent mental illness (SPMI) to:
  • Identify their mental health and other service needs, develop their personal goals, and access needed services including:
    • Mental health
    • Medical / physical health
    • Financial
    • Vocational
    • Social
    • Housing
    • Transportation
  • Advocate for their service needs
  • Educate themselves on management of their illness

*Adult Mental Health Case Management Services are also referred to as:
  • Rule 79 Case Management Services
  • Targeted Case Management Services

Intake Services

Adult Mental Health Case Management Intake Services assist individuals requesting Adult Mental Health Case Management services by:
  • Working with individuals to arrange a diagnostic assessment to establish their eligibility
  • Providing interim case management services to individuals seeking Adult Mental Health Case Management eligibility
  • Providing individuals with information on mental health and other community resources
  • Assisting individuals with referrals for needed services
